How to fill the DACWR (Dental Assistant Course Work Report)

Step 1: Fill out your personal information

You have to fill out your personal information on the first page of the DACWR (Dental Assistant Course Work Report). This will include your name, mailing address, phone number, email address, and which international credential agency used (IQAS/ICES/WES). Ensure this information is accurate and up-to-date, as this is how the regulatory body will contact you.

Step 2: Fill out your education information

The education section (Section A on page 2) of the DACWR form will require you to mention your degree, as well as the institution where you have completed it. You will also need to mention the date issued, the length of the course as well as the entrance requirements of that course (for example, if you have got BDS admission through some entrance test, you can mention it in the entrance requirement column).

Step 3: List your clinical experience

The clinical experience section (Section B, pages 2-11) of the DACWR form will require you to tick the experience you have gained.

You will need to list the document’s name in which the course is mentioned and the year the degree is completed. The DACWR report checks whether you have learned the required syllabus according to the Canadian standard of education.

So, while filling out the Course Work Report, ensure to tick everything in the curriculum to be eligible for the license. If your International degree has not covered the enlisted curriculum, you may not be eligible to apply for the NDAEB Licensure.

For example, let’s take serial no.1 under section B – “Conduct Appropriate to a Professional setting”, this includes principles of ethics, principles of dental jurisprudence, and legal scope of practice. These topics are covered in the final year of BDS Public Health Dentistry. So you can tick those and fill in “Public Health Dentistry” in the “Course title” column. Under the “name of document” column, you can write it as “final year BDS Examination,” and you have to fill in which year that particular course is completed.

In serial no.9 under section B, you need to provide a glimpse of what you have done during your internship.

Step 4: Review the DACWR report

Once you have completed the above steps, you will need to read the instructions and sign the candidate declaration on page 12 and consent for personal information on page 13.

Step 5: Submit your DACWR

Finally, you will need to submit the completed DACWR form to NDAEB. If you have used either ICES or WES as your assessment agency or if you are submitting proof of eligibility from NDEB, you must submit the DACWR to NDAEB directly along with the other required documents. If you have used IQAS for credentials assessment, you must submit the DACWR directly to IQAS for review. IQAS will forward your DACWR to NDAEB with the assessment report.
